ConnectBH (“Connect for Better Health”) is a community access program for non-emergency, non-hospital services such as counseling, nutrition, coaching, and related support programs. ConnectBH is administered by MDHIN (Medical Health Inclusive Network), which manages enrollment, member status, and program operations.

1) Important notices

• Not insurance. ConnectBH is a membership program and is not a health insurance product.
• Not for emergencies. If you have an emergency, call 911 or go to the nearest emergency department.
• Services vary. Availability may depend on your state, provider availability, and program rules.

2) Membership, tiers, and visits

Your subscription tier includes a defined number of covered provider visits per membership term. Visits are intended for eligible ConnectBH services and must be scheduled and completed according to program guidelines. Unused visits do not roll over unless expressly stated in writing.

3) Enrollment and eligibility

By enrolling, you confirm that the information you provide is accurate and that you are authorized to enroll yourself (or the member, if enrolling on their behalf). We may request additional information to confirm member identity, eligibility, and to support program operations.

4) Payments, renewal, and cancellation

Subscriptions are billed on a recurring basis. By subscribing, you authorize recurring charges for your selected tier until you cancel. You may cancel at any time; cancellation will stop future renewals. Fees already paid for the current billing period are generally non-refundable unless required by law or explicitly stated in a written policy.

5) Provider services and clinical responsibility

ConnectBH providers are responsible for the professional services they deliver. ConnectBH and MDHIN do not provide medical advice through this website. Any recommendations or services provided by a participating professional are their responsibility.

6) Acceptable use

You agree not to misuse the program or the website, including attempting unauthorized access, interfering with normal operation, or submitting false information.

7) Privacy

Our collection and use of information is described in the Privacy Policy.

8) Changes to terms

We may update these Terms & Conditions from time to time. Updates will be posted on this page with an updated effective date.
